Cuarenta y tres' is Spanish for 43. It stands for 43 natural, secret ingredients. To this day, there are only three people who know what those ingredients are, and Licor 43 is still produced exclusively in Cartagena. Of course, with good tasting, some ingredients can be guessed, including citrus and vanilla, but we will probably never really know.
